Eliminated the hard-coded values for parameter selection. They are now dynamically generated in the JavaScript.

Shared the Web App in ArcGIS Online with the "JavaScript Working Group", so that we can all use the same appId. Added note that one might need to add their local hosting location for the app, if it isn't already in the list of URIs.
